They are measured  by the inside diameter or the size of the hole after setting. Eyelets and Self Piercing Grommets have a smaller diameter "Face" or "FLANGE" then   standard "sheet metal" grommets. Eyelets  are more commonly sold without a washer but, washers are made and are available for eyelets and do help to assure that the eyelet is secure on the material . Self piercing grommets and Standard "sheet metal" grommets are  always sold as two pieces;  the top grommet/eyelet style piece and a washer  which is placed on the bottom of the project to sandwich the material between.
